Both are men; the difference is where the road started — assigned male at birth or assigned female — not where either of you stands now.

The short version

A cis man was called a boy at birth, and man still fits. A trans man was assigned female at birth and is a man. That is the entire difference, and it is about the starting point rather than the destination. A trans man's manhood is not a lesser copy of anyone's.

From inside, the two feel very different. For most cis men, gender is the thing nobody examines: no story about becoming a man, no words ready, because nothing ever forced them to find any. For most trans men, the recognition is a noise stopping — a low hum they assumed everyone lived with, going quiet when someone finally said the right word.

How to tell them apart

The button. Offered a switch that would make you a woman permanently, no cost, no fallout: a cis man's no is immediate and easy. If the no takes a long time and turns out to be about your job, your marriage or your parents rather than about you, keep reading.
Puberty. Awkward, spotty and humiliating is the cis version. Something being done to you that you would have stopped if there had been a way is the trans one.
Being read as a man by a stranger. If it lands and vanishes like being handed the right coffee, that is cis. If "mate" quietly improves your whole day and the apology afterwards drops something through the floor, it is not.

Which one is more likely

More likely Cisgender Male if…

  • Someone calls you sir and nothing happens. No lift, no flinch, no correction running quietly behind your face.
  • You have never seriously tried out a different name for yourself. Pseudonyms and jokes do not count.
  • You have no story about becoming a man. You just were, the whole time.

More likely Transgender Male if…

  • There are years you have almost no photographs from, and you know exactly which years.
  • You have said a different name out loud in an empty flat, just to hear how it sounds in your own voice.
  • When you picture yourself at forty, the person you picture is a man, and it feels like the version of the file that is not corrupted.
When both words fit
  • Rarely both, by definition: one word describes where you started. What is shared is "man", and it is the word most trans men want in daily life, with "trans" only when it is relevant.
  • Where the two meet is in doing manhood badly and staying men. Camp men, soft men, men who cry in car parks, men who have never held a drill. Neither page revokes anyone's membership for that.

Nobody signs up for years of medical gatekeeping and being a national talking point to get taken more seriously in meetings. A trans man is a man. The direction of the road does not change the word.
“If a cis man has ever wondered about being a woman, he must be trans.”
Wondering is common. What matters is what the wondering does next — whether it leaves, and whether it brings curiosity or relief. A passing "huh" and a quiet "please" are different things.

The useful questions are whether it leaves, whether it brings curiosity or relief, and whether this is the first week or the fourth year. A blank feeling about being a man cannot settle it in either direction, so do not let anyone tell you it does.
